App-op-recept: Vertigo Training

Vestibular rehabilitation is a safe and effective exercise-based treatment for patients with chronic vestibular symptoms. However, it is underused in general practice. Internet-based vestibular rehabilitation (Vertigo Training), which has proven to be effective as well, was developed to increase uptake.

Internationale Inzet

This study will provide much needed insights in disease trajectories of the different subgroups of patients with PF. Simultaneously, the I-FILE study will yield valuable information on the use and feasibility of home-based data collection. This international patient-led registry will facilitate trans-border collaboration to further optimize care and research for patients with PF.

Telemonitoring vs Regular Care

The idea behind using MPVC is that MS patients will gain more insight into the individual course of the disease and get a better grip on their symptoms. This knowledge should increase their autonomy, give patients more control of their condition and enable them to better and proactively interact with health care professionals. As the consulting process becomes more efficient with the use of MPVC, MS-related problems could be detected earlier, enabling earlier multidisciplinary care, treatment or modification of the treatment. This could have a positive effect on the quality of life for both the MS patient and his/her partner/informal caregiver, reducing health and social costs.

Telemonitoring bij Post-Covid

Six months after hospital admission for COVID-19, patients’ lung function and quality of life were still improving, although fatigue persisted. Home monitoring enables detailed follow-up for patients with COVID-19 at low burden for patients and for the healthcare system.

Eenvoud in PROMs

VAS scores significantly correlate with validated PROMs in IPF and are reproducible over time. Because of their simplicity, visual analogue scales have the potential to be used for systematic evaluation of disease course in trials and daily practice.

Due to these specific risk factors, it might imply that behavioral aspects are pivotal in primary prevention of Post-Covid.

Hoge Adherentie en Satisfactie

Home spirometry using an online home monitoring application is feasible in patients with SSc-ILD, with high adherence and patient satisfaction. Larger long-term studies are needed to assess whether home spirometry can detect the progression of ILD in patients with SSc.

Transmurale Samenwerking

The ePHE in our study is the first step in overcoming these barriers. Nevertheless, this strategy needs further development because of the large number
of “lost to follow up” and missing data. Adding a coordinating case manager to this personal digital environment strategy may be of additional value. Another option could be a financial incentive for general practitioners, providing specific education for professionals and introducing ePHE in the management and transition of care.

Hybride Zorg is Effectief

It is shown that COPD patients used the platform more frequently in a blended care setting compared to patients who used the eHealth platform independently, adjusted for age, sex and disease burden. Blended care seems essential for adherence to eHealth programs in COPD, which in turn may improve self-management.

Our experience is that home monitoring of lung function, symptoms, and adverse events can secure patient safety from both a care and clinical trial perspective. At the same time, patients are more actively involved in their disease, enhancing self-management.

The present study showed that a change of at least 3.5 points in the patients’ SFNSL score represents a clinically important difference, in that the SFN‐related symptoms showed a relevant increase or decrease. Some caution is warranted for the use of the MID in individual cases in the clinic, as the SDC was found to be higher than the MID.

RCT Bewijst: Individueel Maatwerk via Thuismonitoring

The results of this first-ever eHealth RCT in IPF showed that a comprehensive home monitoring program did not improve overall HRQOL measured with K-BILD but tended to improve psychological well-being. Home monitoring was greatly appreciated by patients and allowed for individually tailored medication adjustments.

Furthermore, a lack of effect on empowerment is not because of nonusage of the program components.

Patient experiences and satisfaction with antifibrotic treatment were fairly positive, and similar for nintedanib and pirfenidone. Systematic evaluation of patient expectations, experiences, and satisfaction with medication could enhance shared-decision making and guide drug treatment decisions in the future.

In conclusion, a comprehensive home monitoring program is feasible and can be used in sarcoidosis research. Home monitoring could also be attractive for use in daily care, though studies are needed to evaluate its role and additive value. Potentially, home monitoring may enable timely recognition and response to changes in symptoms, lung function, and activity. Especially in a heterogeneous disease such as sarcoidosis, home monitoring may pave the way for better individually tailored treatment, enhanced self-management, and improved quality of life.

A home monitoring program including wireless home spirometry, is highly feasible and appreciated by patients with IPF, and enables real-time detection of change in FVC and PROs facilitating personalized care.

Hoe ontwerp je goede eHealth?

All together, we believe that the use of IPF-online is feasible and highly valued by patient with IPF. Whether its long-term use improves QoL, medication use and end-point assessment for trials is a field for further studies.

Internet monitoring of pain is feasible in most outpatients with cancer-related pain. The frequent use of the pain diary in the majority of patients indicates that those patients felt confident with the regular assessment of pain.

Pain and itch are highly prevalent months after HZ. Pain caused by HZ has a large impact on quality of life, burden of illness, impact on daily life, and health care costs for these patients. The impact of itch on quality of life is relatively small.

There are indications that neurosarcoidosis is insufficiently recognized as a separate clinical entity. In the absence of major prospective clinical trials, we launched a large national online registry of neurosarcoidosis patients in the Netherlands in June 2014 at www.neurosarcoidose.nl. When a patient with sarcoidosis presents with neurological symptoms that may compatible with sarcoidosis, it is important to locate sites of disease activity accessible for biopsy outside the nervous system.

The observation that three out of four (75.5%) RRMS patients completed two short questionnaires at all monthly time points during 1 year after the start of disease modifying treatment suggests that intensive online monitoring in this patient group is feasible. As only one in five (19.6%) patients adhered to the time intervals between self-assessments, measures are needed that improve the timely completion of questionnaires.
